Goupile is a free design tool for secure forms including Clinical Report Forms (eCRF). This project is funded by the NGI0 Core grant.
With Goupile you can easily create complex forms and share them with colleagues and users to collect data, there is access control available in the administration module which allows managing project and users, allowing them to fill out the data and export it. The forms are javascript files which run on the server, and it provides a secure sandbox feature on linux built on seccomp filtering which isolates the js runtime.

Try it out
